Acceptance/Admission criteria: There is paperwork to fill out before mental health appointments can be made for counseling. The answers from such paperwork will help decide if criteria is met to make an appointment. From there, the individual will be contacted. For Community Home Based Services (hourly or daily); An application for services must be complete and can also be picked up at our office, or you can email Angie Torres for such information (email above)). We require proof of Mental Health Diagnosis from a medical professional and funding from a Case Management Program (Integrated Home Health, Des Moines County Community Services, Southeast Case Management Services, etc.). We are able to refer to such Case Management Services as needed to establish funding, whether an individual has insurance or not. Payment options: Medicaid, Medicare and all private insurances. We provide Mental Health Counseling in our Burlington, Mt. Pleasant, Keokuk, and Ft. Madison Offices. Our Burlington office number can direct calls as needed for specific locations. We also provide Community Home Based Services where we provide a skill building service to customers with a mental illness or disability. Such skill building may include; medication management, money management, stress management, community integration, etc.

LifeLong Links gives you access to: housing, healthcare, transportation, employment, mental health, respite, food/nutrition, education, financial assistance, and adaptive equipment to mention a few.
